Actually the name "Bochenek" is not mentioned, only "Yakev Dov son of R. Yosef", or rather "our friend/comrade Yakev Dov, son of our friend/comrade R. Yosef".
We have not managed to read the document. The phrase "our friend Yakev Dov" occurs five times. Also "b-chai v-b-mevet", apparently. And of course there is a bit of hand-printed Hebrew at the end. At the bottom there are two signatures (on the right: "signed"; on the left - the signatures).
Family tradition states that somewhere within this document it is stated that Yakov is a good cobbler.
